What are bioidentical hormones?
Bioidentical hormones are an alternative to traditional synthetic hormones. They are compounded in the lab to have the same molecular structure as the hormones made by your own body. Synthetic hormones are intentionally different because drug companies cannot patent a bioidentical structure. Instead they invent synthetic hormones that are patentable. Some examples of these are Premarin, Prempro, and Provera.
There are several branded versions of bioidentical hormones therapies available for use. The Wiley Protocol is the brand promoted by Suzanne Somers and Oprah. We offer this to our patients who have researched bioidentical hormones and decided this is for them. Many of our patients needs are met by an individualized approach rather than a one-size-fits-all dosage regime. In our practice we begin with laboratory tests of hormone levels. When warranted, we prescribe a precise dosage of bioidentical estrogens, progesterone, testosterone, and DHEA that is prepared at a registered compounding pharmacy. Each patient is then monitored carefully through regular follow-up hormone panels to ensure she gets symptom relief at the lowest possible dosage. Initially we do a hormone panel every three months. Once balance is restored, we do one a panel each year at the time of the annual exam unless the patient has problems before then.
Are bioidentical hormones better and safer than synthetic hormones?
Bioidentical hormones are better and safer than synthetic hormones but they are not perfect. Bioidentical hormones are natural and our bodies can metabolize them with minimal side effects. Synthetic hormones often produce intolerable side effects and by being mass produced cannot be customized for each individual the way compounded bioidentical hormones can. Studies suggest that bioidentical hormones are safer than synthetic versions, but here have not been long term use studies that prove they are completely safe.
Let us note here that the WHI studies on the effectiveness and health risks of HRT were based on synthetic/equine-based estrogen and a progestin instead of progesterone. The studies were also flawed because the average age of the women in the study was 63. Without BHRT by age 63 most hormone related diseases have already established a foothold. Another factor learned is that oral estrogen increases clotting risks which causes more strokes and heart attacks. We now know that the sooner BHRT is begun the more effective BHRT is at preventing these diseases.
Is BHRT the answer to my problems?
Many things influence our hormones, namely our diet, lifestyle, and the environment. It is important to look at these issues before turning to bioidentical hormones for all the answers to our problems. In our experience, the majority of people can improve hormone imbalances with sufficient exercise, getting enough proper nutrition, and reducing stress by making changes in their lifestyle. This is the first step to healthy aging.
